How can the spring statement affect doctors and NHS Healthcare professionals?

Chancellor Rachel Reeves has delivered her latest Spring Statement—and while it may not have made headlines for major giveaways, it could quietly hit your wallet.

If you’re a doctor working in the NHS or running a private practice, here’s what you need to know—and what you can do to prepare.

💣 First, a Quick Recap from Previous Budgets (Still in Effect)

Some key changes from the Autumn budget (aka “The £40 Billion Tax Grab”) are still rolling on:

  • Tax thresholds are frozen until 2028—dragging more NHS doctors into higher tax bands without raising your pay.

  • Inheritance tax thresholds are frozen until 2030—not great news for anyone building long-term wealth or estate planning.

  • Stamp Duty Land Tax cuts—ending 31 March 2025.

  • National Living Wage—rising to £12.21/hour in April (great for staff, but tough for clinic owners and contractors).

  • Employer NIC increase—1.2% to 15% hike from April 2025.

  • Business Asset Disposal Relief—going up to 14% tax.

  • Non-dom tax regimescrapped from April 2025.

  • VAT on private school fees – already come in from January 2025.

  • Furnished Holiday Let’s relief—scrapped from April 2025 (bad news if you’re a doctor with property investments).

Here’s what’s just been announced—and how it could affect you:

🧾 Taxes

  • No new personal tax cuts. Income tax thresholds still frozen. That’s stealth taxation at work.

  • More funding for HMRC to pursue tax evasion. So, getting your tax affairs in order has never been more important.

  • High Income Child Benefit Charge – from Summer 2025, you can pay this via PAYE (helpful if you’ve been caught in this trap and missed claiming child benefit).

  • ISAs under review – No changes yet, but the government is looking at balancing cash vs stocks ISAs. (Hint: most doctors are still overloading on cash and losing to inflation.)

💷 Spending & Cuts

  • Welfare budget slashed by £3.4bn.

  • Civil service cuts coming – with a £3.25bn “transformation fund” to reduce government overheads.

  • Defence spend rising – including drones and AI, reaching 2.5% of GDP by 2027.

  • Heathrow 3rd runway backed and Portsmouth naval base regeneration announced.

  • Construction sector boost – £600m pledged to train 60,000 new workers.

  • Triple lock protected – state pension and pension credit rising by 4.1% in April.

  1. You may already be in a higher tax band without realising it—especially if you earn between £50k–£150k.

  2. Your tax burden is rising—even if your salary isn’t.

  3. Private practice owners and locums will feel the pinch from rising employer NICs and regulatory costs.

💡 What Should NHS Doctors and Medics Do?

  • Review your tax efficiency—many doctors are still missing basic reliefs on professional fees, pensions, and investments.

  • Plan around the April 5th tax deadline—especially if you’re in the 60% tax trap (£100k–£125k earners).

  • Get ahead of ISA and SIPP changes—use your allowances now.

  • Don’t sleep on estate planning—inheritance tax isn’t just for millionaires anymore.
