Starting your career as a GP is exciting, but negotiating your first GP contract can make the difference between financial freedom and unnecessary loss. Surprisingly, many doctors, especially newly qualified GPs lose thousands of pounds each year simply because they don’t negotiate.

It is not about your clinical skills or dedication. It is the small, overlooked details in your first contract that quietly add up over time. Knowing what to ask for and when to ask is critical to setting the right foundation for your career.

Why Many Doctors Avoid Negotiating

Many doctors go through training with a mindset like this:

  • “I want to be a team player.”

  • “I want to be agreeable.”

  • “I don’t want to seem like I’m asking for too much.”

  • “I’m scared they might withdraw the offer if I push back.”

While understandable, this hesitation can be costly. In my experience, taking the time to negotiate early can mean the difference between coasting financially and starting your career on strong footing.

My Experience With Negotiating a First GP Contract

When I negotiated my first GP contract, I discovered:

  • Flexibility to build a portfolio career

  • Time to pursue projects, including a fellowship designing a service for my county

  • Willingness to accept slightly less pay in exchange for support and flexibility

I turned down my first three offers, even those with higher salaries. One offer seemed like a no-brainer – visa support, competitive pay, but the contract contained phrases that didn’t align with my long-term goals. Consulting a senior GP confirmed my concerns: the contract wasn’t bad, but it wasn’t tailored to the career I wanted.

This was an eye-opener. I hadn’t realized how much could be discussed or how costly it would be not to.

You Are Valuable, Don’t Undervalue Yourself

Remember:

  • You have had over a decade of training.

  • If you are a GP, you are a family medicine specialist in high demand.

  • If they didn’t want you, they wouldn’t be offering a contract.

Negotiating is not selfish, it is professional. It ensures your career starts with the right conditions and pay.

Key Areas to Negotiate in Your First GP Contract

Here are the main areas to focus on before signing:

  1. Salary – Ensure it reflects your value and market rates.

  2. CPD Funding & Study Leave – Support for continuing education is crucial.

  3. Relocation Allowance – Important if moving to a new region.

  4. Protected Admin Time – Prevent clinical hours from creeping up.

  5. Golden Handshake / Recruitment & Retention Premiums – Especially in shortage areas.

  6. Flexibility on Sessions – Align with career, family, or portfolio work.

  7. Annual Leave Entitlement – Check exact days and public holiday handling.

  8. NHS Pension or Alternative Workplace Pension – Confirm contributions, eligibility, and voluntary options.

  9. Exit Conditions – Notice period, clawback clauses, and restrictions.

  10. BMA Contract Checking Service – Highly recommended before signing.

Financial wellbeing starts with a solid income, but good working conditions are often the foundation. Tip: your first job offer is the easiest time to secure these benefits. Renegotiating later is much harder.
